Medical Assistance Grant Program
Here's a great program for individuals who need assistance paying for their medical requirements. Easter Seals of the Birmingham Area has a Medical Assistance Grant Program for individuals with disabilities. The program was created to assist in paying for medical requirements of children and adults who have disabilities and are unable to provide for their own needs. For consideration of funding, applicants must complete an application and include the following required documentation: • Proof of disability • Proof of need as related to the disability from a referral source such as: a qualified agency, qualified professional or a physician. All information must be current (within one year). • Written estimate regarding the item(s) requested. Information must include the name of the vendor and prices. Listed below are items that are not ordinarily covered by the Easter Seals Medical Assistance Program: • Bills of any kind • Equipment already purchased • Respite care • Hearing aids • Prescription and non-prescription medication • Ongoing treatment/therapy • Surgical procedures • Diagnostic testing • Eyeglasses • Doctor visits • Vans/Automobiles Assistance is limited to the availability of funds. Requests exceeding $500 cannot be considered. Easter Seals of the Birmingham Area serves the residents of Jefferson, Shelby, Walker, Blount and St. Clair Counties.
