Supportive Housing

Are you in need of housing? The following program provides housing and assistance to individuals and/or families in the New Orleans, LA area: Obtaining and maintaining adequate housing can be challenging for individuals with disabilities and their families. In an effort to assist in securing and maintaining proper housing, the State of Louisiana administers the Permanent Supportive Housing program. Through the Permanent Supportive Housing program (PSH), individuals are provided with supportive housing in an effort to prevent them from becoming homeless or institutionalized. Individuals who are eligible for the PSH program are also paired with a housing support team who offers a variety of supportive services based upon their individual need. To be eligible for housing and supportive services available through PSH, individuals must meet the following three (3) requirements:

  • Households must earn less than 30% of the area's median income
  • Individuals must demonstrate a need for housing support services
  • A member of the household must have a substantial, long-term disability as determined by the Department of Health and Hospitals

The PSH program is provided through six (6) Department of Health and Hospital Regions of the state. For Orleans, St. Bernard and Plaquemines parishes, the PSH program is administered by the Office of Aging and Adult Services. The following organization offers a variety of social services to the families and individuals in the Los Angeles, CA area: Throughout the Greater Los Angeles area many families and individuals are feeling the effects of a tough economy. Faced with financial hardships, many individuals are left homeless or near homeless with no place to go. Does this reflect your situation? If so, look no further. Thanks to the St. Francis Center, many families and individuals are receiving the services they need to get back on their feet and into the community. The St. Francis Center is a nonprofit multi-service community center dedicated to meeting the immediate needs of underserved families and individuals in the Greater Los Angeles area. The Center's sole purpose is to offer quality services that educates, empowers and provides individuals with the tools that are necessary for them to build better lives. Services offered through the Center include:

  • Pantry Service - Provides families and/or individuals with grocery bags filled with fresh fruits, vegetables, meat, dairy, rice, beans and canned food.
  • Senior Services - Offers seniors the opportunity to socialize with peers and participate in the "Senior Brown Bag" program which offers workshops on a variety of subjects.
  • Homeless Services - Through their homeless services, the St. Francis Center operates a soup kitchen and provides mail services, health and hygiene services and respite services during select periods.
  • Youth Services - Provides educational and recreational youth programs to empower the success of low-income youth.

In addition to these services, the St. Francis Center also provides case management services and host community forums that address family and community issues. The Center also host medical clinics and provide medical access through partnerships with other agencies. Helping Families

The following organization provides assistance to families in need throughout South Mississippi in the Biloxi, MS area:

Recently I learned of an organization that provides help to families in Southern Mississippi during their time of need. Throughout the years the mission and services of Hope Haven Children's Services has undergone several changes. However, their primary mission of helping children grow up in loving and safe homes remains the same. In addition to providing emergency shelter to abused and neglected children throughout Southern Mississippi, Hope Haven also provides assistance for families experiencing a financial crisis.

Through their Helping Hands Program, Hope Haven focuses on keeping families together by offering assistance to families with children up to the age of 18. Assistance offered through the program include (but is not limited to): clothing assistance, help in paying past due rent and/or utility bills, assistance in purchasing medications and gas to get to and from work.

Do you need assistance in paying your phone bill? The following programs provides discounts on phone service to eligible individuals: To ensure that telephone services remain available and affordable for low income consumers the government has established the Lifeline and Link-Up telephone assistance programs. Lifeline and Link-Up are programs that ensure that quality telephone services are available to low-income consumers at affordable rates. These programs provide discounts to eligible households for both the initial installation (Link-Up) and monthly phone bills (Lifeline) toward local service. Link-Up: Link-Up helps low-income families reduce the cost of initiating telephone services. The program will pay half of the installation charges for new local telephone service to your home or the activation of a wireless phone service up to a maximum of $30.00. However, the Link-Up discount does not cover charges related to wiring and telephone jacks within the home.  Lifeline: Lifeline offers low-income families a discount on their monthly telephone bills. Through Lifeline families can save up to $13.50 per month on the local service portion of their telephone bill. This program does not assist with long distance or other special feature services.

Who is eligible for Link-Up and Lifeline? The qualifications for both Link-Up and Lifeline are the same:

• If you are currently enrolled in one of the following public assistance programs, you are automatically qualified for the telephone assistance program:

oSNAP (Food Stamps) oMedicaid oSSI (Supplemental Security Income) oTANF (Temporary Assistance for Needy Families) oLIHEAP (Low Income Home Energy Assistance Program) oNational Free School Lunch oParticipate in Section 8 Housing/Federal Housing

• If you are not enrolled in any public assistance programs you may still be able to qualify based on your household's gross monthly income. To be eligible, applicant's total household income must be equal or less than 135% of the Federal Poverty Line. For more information about Lifeline or Link-Up services, contact your local telephone company, your state public utility commission or consumer advocte office to ensure that you qualify for the benefits of Lifeline and/or Link-Up or request an application form. Accessible Transportation

Are you unable to use public transportation due to a disability? Try AccessRide!  AccessRide serves as the ADA Paratransit Service for the Nashville Metropolitan Transit Authority (MTA) . ADA Paratransit services are provided to individuals who, because of a temporary or permanent disability, are unable to independently use public transportation. To be eligible for Paratransit services, individuals must complete an application and demonstrate that because of their disability they are unable to use MTA's fixed-route bus service for one (or more) of the following three reasons:

  • Unable to independently ride a bus - This applies to those who cannot board, ride or exit a bus on the fixed-route system
  • Unable to get on or off a bus - This applies to individuals who cannot, due to floor and/or lift equipment, board or exit a bus on the fixed-route system
  • Unable to get to or from a bus - This applies to individuals who cannot access a bus stop to board a bus on the fixed-route system and cannot access their final destination after exiting from a bus on the fixed-route system.

To receive an AccessRide application, interested applicants must call (615) 880-3970, ext. 1104. Once the entire application, including the Professional Certification, is completed, individuals must call to set up an in-person interview and functional assessment. During the interview, an eligibility specialist will review the application and/or assist in the completion of the application if necessary. Applicants may be asked to participate in a functional assessment and must bring their primary mobility aid or aids that they would use when traveling in the community. If necessary transportation to and from the interview will be provided at no cost to the applicant. To schedule an interview, applicants must call (615) 880-3596.
