Apply for Social Security Disability Benefits
You may apply for benefits online at www.ssa.gov/applyfordisability/adult.htm, or you may schedule an appointment with your local Social Security office and they will help you in person or by phone. If you work with your local office you will need to complete a printed application for Social Security Benefits and an Adult Disability Report.
Before beginning your application, it is best to print and complete the Checklist – Adult disability Interview and Medical and Job Worksheet. These forms will help you collect the information needed for the application.
If you are applying for a child, you will need to call Social Security and schedule either a phone or in person appointment to apply. Applications for children cannot be completed online.
